  To facilitate your consultation with Dr. Allam we encourage you to print and fill the information needed and bring the forms with you. 

  There are three forms to be filled:

Directions:
Sunrise Highway-POW/MIA MEMORIAL HWY/NY-27 East toward MONTAUK.
You will come to an intersection with a 7/11 on the right, "upper" corner.
Keep going straight through 1 more light.
Get into the center lane.
The building is a brick 2-story, on the left as you are heading east,
just past B&N Furniture and Sleep Center.

Directions
LI Expressway east to the last exit.
Merge onto Rt 58 and follow to the traffic circle.
Follow the circle around to Roanoke Ave
(between 7/11 and Central Suffolk Hospital)

3rd Tuesday of each month, excluding December~6:30-8:30pm
 

From the West:
Take the Sunrise east to exit 65 South, the Hampton Bays exit. *The exit ramp leads into Route 24; continue south to the traffic light. The Hampton Bays Diner is directly ahead. Turn left at the light onto Montauk Hwy. Follow this into Hampton Bays, turning right at the third traffic light onto Ponquogue Ave. Follow this 1/2 mile to the Hampton Bays Public Library, on the right at the corner of Ponquogue and Argonne Rd. West. Parking and entrance are in the rear of the building.

From the East:
Take the Sunrise west to exit 65. Follow directions from * above

From the North Shore:
Follow Route 24, Old Riverhead Road, south to the Hampton Bays Diner. Follow directions above from * above.
